While I can't speak to Hollywood-styled movies, for an independent producer like myself, the reasons are two: cost and access. I could not afford to hire a horse for a scene, and shooting outdoors with the horse (i.e. getting a good location for that) would be challenging. Plus, I'm guessing most of the models I work with do not have experience riding with horses.

Now, I have read a number of news stories about riders and their horses getting trapped in the real thing...and anyone can Google those.

I recall, rather vaguely, that MPV did one a LONG time ago, but Michael nails the answer. When a model, or reasonable facsimile thereof, comes to my Studio with a horse and saddle we will shoot such a scene.

you probably would not want to use an animal because of humane issues. like movies and tv shows say it was monitored and none were harmed while making the show. it would be a chore to get the animal out.

As you can't sink the horse for obvious reasons it would only be kind of a prop to introduce the scene. Renting a horse just for this would gather almost no additional sales. Using a horse in a scene (like the rider can use her horse to pull herself out or something like that) may be possible and interesting for sure, but would be much, much more expensive.

As long as you don't want to pay a really big amount of money for such kind of custom scene it will not happen. Better spend the money on the actress and her attire (high-quality riding-clothes are expensive, not to mention the boots!)
I'm already chindeep in this mudbog and every desperate attempt to move my stuck legs only drives me deeper in. The thick mud slowly swamps my waders and my arms have nothing to hold onto.
I'm feeling home.

Bogbud is quite correct here. One of the actresses I have used quite a bit for the past five or six years is a VERY skilled rider. Skilled enough, she has said, to ride a horse standing up. She does not have her own horse however, and even if she did getting the horse here would be prohibitively expensive. Other than as a very expensive custom scene, therefore, about the only way such a scene could happen would be if there was a local model (and I mean with five miles or so) who decided to ride her horse to the Studio. There ARE people who own horses within two miles but none, so far as I am aware, even remotely fit the profile of what we would call a model.

I know of one but its older, its an MPV scene. I actually have it on VHS tape and the model is *Jen* you actually see her riding her horse and I love it.This must date back to the 90,s and secretly hoped *Summer* could have made a re-make but surely this could be a challenge. Also lets not forget Lena,s equestrian scene, she isn't riding a horse but has that perfect outfit...nothing hotter than seeing a woman wearing tight jodhpurs.

Lena Ramon, The Horserider.

Very good scene in thick stuff. She also wears rubber riding boots. That's a rare sight nowadays.

I do know of two scenes in which there is a horse paying a major role but who is not seen.

The old muddygirlies.com website used to have a preview section where you could see 4 frames from every movie they made. I never invested in any of their stuff but I distinctly remember one available video where they had the rider ride past a mud pit before jumping off the horse/"being thrown" into the mud. It looked like a pretty gorgeous scene with some reasonably deep mud judging from the shots I saw.
However, the website has since been redesigned and the catalogue reduced so I can't point you any further than that.
