I need help identifying the missing song lyrics.

I heard the following song a few times on an internet radio station. It's a great song! I like the singer's voice! This song is special to me for reasons. I did some extensive research but couldn't find the lyrics online. After listening to the song several times, I was able to somewhat piece together most of the lyrics, but I'm uncertain of what is being spoken during certain parts. I will insert a link to the video and write down the lyrics I was able to come up with on my own. Wherever "[unknown]" is written is what I couldn't understand by listening. Maybe some of you have better listening skills than I do! lol!

I appreciate any help I can get. Thanks in advance for the effort! Feel free to correct my errors! I love this song!
