Awhile back in Oneida, NY a bus driver went under one of those- with a double-decker bus.
The poor bastard who was sitting in the front of the upper deck. . . ruined his whole day. And then there are the Parkways in NYC, which are closed to all commercial vehicles- and drivers blindly follow their (non-commercial) GPS, ignore the warning signs, rip down the cable-mounted clearance signs at the entrances, and promptly get stuck under a low bridge.
Sometimes the bridge comes out second-best. I think it was in the Midwest someplace- normal clearance bridge (13' 6" at least) but some (fill in your own insult/slur) was hauling a huge excavator. He must have been going like the milltails of hell, too. Took out three of the prestressed concrete girders on the overpass. Luckily it didn't collapse, but it had to be replaced.
A former coworker of mine pulled this one. She was driving a DOT snowplow, doing her job during a snowstorm. She got to the end of her beat, reloaded with sand at the reload point, and started back. There was a car spun out, so she had to detour a bit. This took her through a very small underpass. She knew her route, knew the height and width of her truck, so she knew she could get through the underpass. But, she forgot about the nice high heap of sand on top of the spreader!
Next car to come through the underpass got high-centered on the sand and had to be towed out.
